This course provides an opportunity for learners to complete their evidence-based practice (EBP) project proposal that addresses a problem, issue, or concern in their specialty area of professional practice. Learners previously identified a problem amenable to a research-based intervention, searched the literature, and proposed a solution. Now learners will explore implementation considerations and various evaluation methodologies, complete the project proposal by developing a plan to implement the solution into the intended practice area, and design an evaluation plan that will assess the EBP project proposal’s intended outcome(s). Will educating nurses to perform continuous compression during defibrillation with no shock pauses for patients in cardiac arrest compared to no education increase the probability of return of spontaneous. The latest guidelines emphasize that chest compressions should be performed with adequate rate and depth, complete recoils, and minimized pauses (Luo et al., 2021). Pauses during resuscitation have can extended pass the recommended guidelines of 5-10 seconds during defibrillation due to Charging defibrillators prior to analyzing heart rhythms may decrease the no-flow time during rhythm check pauses while resuscitating in cardiac arrest (Kemper et al., 2019). Administering high-quality chest compressions has been considered a key determinant for successful resuscitations (Luo et al., 2021). Devices such as the LUCUS device are capable of providing mechanical chest compressions allowing for continuous compressions to be in progress during shock administration. Educating nurses on the importance of continuous uninterrupted compressions may help eliminate  or shorten prolonged pauses during resuscitation efforts. In addition, educating nurses about technologies available to help decrease pauses during resuscitation, ultimately improving patient outcomes by increasing perfusion.

This topic is important to continue research being that the American Heart Association has recently emphasized compressions as the first response action for patients in cardiac arrest. Compressions allow for perfusion, giving the patient the best chance for survival. As an Emergency Department nurse it is virtual, the pauses are kept to a minimum and human error is eliminated to the best of our ability. Human errors such as equipment malfunctions, equipment use, and overall knowledge that defibrillation can occur simultaneously during compressions are all factors that can affect a patient’s outcome.

Dr. Taylor moved for a summary disposition “because Cindy’s refusal was not conscious, competent, contemporaneous and fully informed.” The trial court found that Cindy’s refusals of a transfusion were made when she contemplated “merely routine elective surgery” and not life-threatening circumstances, and that, “it could not be said that she made the decision to refuse a blood transfusion while in a competent state and while fully aware that death would result from such refusal.” The record apparently reflected “the unexpected development of a medical emergency requiring blood transfusion to prevent death or serious compromise of the patient’s wellbeing.” The trial court therefore granted summary disposition in favor of Dr. Taylor. Scientific management theory is based on the theory that humans can be motivated by money and that there must be separation between managers and workers (GCU 2018). Management functions were, at the same time delineated as planning; organizing, staffing, directing and controlling (GCU2018).When we look at nursing today, these functions basically describe the day to day challenges of healthcare, not only at the management level but at the level of the floor nurse as well. For managers we plan the needs of our units, organize those needs, staff accordingly, and direct the staff via policies and procedure and control staff through accountability and responsibility. At the level of the floor nurse these functions may be as follows; receiving our assignment and planning our workflow, organizing our priorities and assessing and directing support staff to accomplish our tasks and controlling our day through time management. Inefficiencies occur through micromanagement, when managers refuse to allow autonomy and the feeling of ownership for the staff. Another great inefficiency is proper communication. Many times process changes don’t get communicated time or efficiently causing error or omissions that effect patient care, and may even be patient safety issues. At my institution there are multiple committees that are chaired and staffed by floor nurses that suggest process change based on evidence based practices regarding patient care. EMR is huge for us and making the documentation efficient yet compliant is very important to staff where time management is involved. This committee is made up totally of floor nurses and charge nurses. At my institution participation, engagement and ownership are encouraged of all staff. Communication seems to be challenge in health care even though it probably the most import part of it.

We are at a point in our career where management skills are necessary to have. The lecture notes and chapters bring into perspective of what management and leadership should be. Frederick W. Taylor was an American mechanical engineer, efficiency expert, and management consultant (Giannantonio & Hurley-Hanson, 2011). The father of Scientific Management published his influential work in 1911, The Principles of Scientific Management, where he laid out the process to increase worker and organizational efficiency (Giannantonio & Hurley-Hanson, 2011). His principles contributed to a wide selection of 20th century management practices, including task specialization, assembly line production practices, job analysis, work design, incentive schemes, person-job fit, and production quotas and control (Giannantonio & Hurley-Hanson, 2011). Taylor’s theory was to improve an organizations efficiency by using the most scientifically and mathematically ways to complete tasks (Giannantonio & Hurley-Hanson, 2011). This theory focused on areas such as monitoring job performance and the most effective way to perform it, use protocols for training, and using a reward system to improve productivity (Giannantonio & Hurley-Hanson, 2011). He determined if his theory was used it improved productivity and employee morale. The improvements not only helped with the productivity but also with employee and management relationships. Eyre (2010) pointed out the Four Principles of Scientific Management.

Taylor’s four principles are as follows:

1.Replace working by “rule of thumb,” or simple habit and common sense, and instead use the scientific method to study work and determine the most efficient way to perform specific tasks.

2.Rather than simply assign workers to just any job, match workers to their jobs based on capability and motivation, and train them to work at maximum efficiency.

3.Monitor worker performance and provide instructions and supervision to ensure that they’re using the most efficient ways of working.

4.Allocate the work between managers and workers so that the managers spend their time planning and training, allowing the workers to perform their tasks efficiently.

Some examples of participative decision making in my workplace include staff to patient ratios and assignments. Staffing is an issue not only in my hospital but in all of the healthcare field. There is either not enough staff to care for the patients or there are not enough patients to keep the staff on shift. In the mother/baby unit the continuous changes in staff to patient ratios and the constant change in the census of patients makes this are hard to manage. Participative decision making in my unit exists with assigning patients to the staff member based on acuity numbers with the maximum number someone’s assignment can be is ten. There is constant flexibility in the number of patients we take care of, but we can have anywhere from 2-4 couplets (mom and baby) at any given time. The nurses are able to discuss with the charge nurse if their assignment is overwhelming or unsafe then the charge nurse will adjust and accommodate the assignments as needed based on patient acuity. This helps keep productivity up, increases patient safety and maintains an equal balance of fairness. If we were not allowed to adjust accordingly, the other nurses would be overwhelmed and morale would be low, the charge nurse would have to answer why the assignments were not adjusted to help the nurses maintain good productivity and patient safety.
