The role of a nurse is to provide holistic, competent care to the patients and families that we are caring for. My definition of advocacy is speaking up for our patients, families, and ourselves to provide the most optimal safe care. Nurses are advocates for our patients every day. We are their voice when they can’t speak or even when they can speak. We listen to their concerns and questions and we get the answers if we can’t answer them. We advocate for their patient rights, culture, religion, etc. Being an advocate is part of being a great nurse. I take care of a lot of ventilated and sedated patients in the ICU, they don’t have a voice during this time so I have to be their voice. One situation that I remember very well was a post C-Section that came into the Surgical ICU post op that required multiple transfusions throughout the day. She was intubated and sedated and her blood pressure wasn’t stable which required vasopressors. She had delivered a healthy baby with no complications and the baby was placed in the nursery during all of this. This was her first child. As an advocate for her, I pulled in the medical team at her bedside and recommended that once she stabilized if we could get the nursery and the father to bring the baby into the SICU so she could touch and hear the baby. What I was asking and advocating for was also against SICU policy. The team thought it was an excellent idea even though it was against policy but this was in the patients and families benefit. In my mind, that’s all a mother longs to do is hold and touch their newborn baby. This patient didn’t get that opportunity due to her circumstances. Later that evening, the nursery brought the baby in and we placed the baby in her arms. We told the mother that her baby was in her arms and that her husband was there as well. The next day the patient was taken off the ventilator and was doing much better. I feel that me advocating for her and her baby helped her heal. If I didn’t advocate for her she would have never been able to touch her baby when she probably needed it the most.

In statistical hypothesis testing, a type I error is the rejection of a true null hypothesis (also known as a “false positive” finding), while a type II error is failing to reject a false null hypothesis (also known as a “false negative” finding). More simply stated, a type I error is to falsely infer the existence of something that is not there, while a type II error is to falsely infer the absence of something that is (“Type I and type II errors,” 2018). For example, if doing a test for cancer and that test would determine if a cancerous organ should be removed, then you want to set the alpha level very stringent at 0.01. You do not want to remove an organ if it is cancer free. On the other hand, if you set the alpha level too stringent and you determine that the organ does not have cancer, when in fact it does then the patient may die prematurely because you made a type II error, failure to detect a difference when one in fact does exist. This can make for a delicate balance.

Situations where you might raise the alpha to 0.1 can vary greatly. The obvious one is when the results of the study are not that critical. For example, when trying to see if a mood therapy has an effect, you might raise the alpha to find the effect even though this increases the chance of making a type I error. Certain studies may appear to have biased results because they could have raised their alpha to 0.25 to say their product is better, and the statistics will support that claim, however the chance of making a type I error is 25%! This is why it is important to know statistics to make decisions that are important to you.

Generally, the only reason to raise a type I error rate is if a limited sample size or if no money to conduct the study at the 0.05 level. Choosing between a type I error or a type II error is a tradeoff, the more stringent the type I error the greater the change for a type II error. The question then becomes which is the lesser of two evils. Is it better to say there is an effect when there wasn’t (type I error) or is it better to say there was no effect when there was (type II error)? Jessie was a typical 2-year-old who liked to try and spread his wings. He would run and fall throughout the house. On several occasions, with minor falls, a bruise would develop on his legs or arms. Jessie’s parent would often worry about being reported for child abuse because of the easy bruising that Jessie experienced.

At three, Jessie would complain that his ankles hurt. This was especially true after a hectic day of running and playing with other kids at the playground. Jessie’s parents wondered if he was simply trying to get attention. Which he usually did being an only child.
