Isaac has worked as a staff nurse on the telemetry floor for over 15 years. He holds seniority in the unit. His patient care is satisfactory; however, his interpersonal behaviors are becoming an increasing issue for his coworkers. He throws papers around the unit, gives short answers to questions, and seems generally miserable. He tells the staff that they are lazy and stupid. He is constantly questioning their decisions. You have come from another local hospital in the role of the assistant nurse manager. Based on your observations, you have met with Isaac informally and discussed his behaviors, but they have not changed. Now three new nurses have already come to you saying that this unit is a great match for them, except for one problem. Although they have not identified Isaac by name, they have told you that one of the nurses is extremely abusive verbally, and they have been calling in sick on the days they are scheduled to work with this person.

1. What are your responsibilities as an assistant nurse manager in regard to Isaac’s behavior problem?

2. What is the next step in dealing with Isaac’s behaviors?

3. How will you, as the manager, have Isaac develop more effective people skills?

What are your responsibilities as an assistant nurse manager in regard to Isaac’s behavior problem?
2.As an assistant manager in the field of nursing, it is my responsibility to show and implement equally my leadership qualities to the staff members irrespective of their experience in the field of nursing. Isaac’s behavior is a challenge that I am faced with since the behaviors affect not only affects the some of the daily nursing activities but also affects the other staff members particularly when he verbally uses the abusive language. In some situations, leadership in nursing needs creativity in solving certain problems such affecting the staffs such as the one inthis case[AJu13]. In addition, a nursing leader should also address all the leadership principle on the issues affecting the staff members or the people they lead. Based on the consultative theory

2. What is the next step in dealing with Isaac’s behaviors?

3. How will you, as the manager, have Isaac develop more effective people skills?

This is your first position as a nurse manager. The holidays are rapidly approaching, and the hospital policy states that each unit will negotiate holiday coverage individually. You are already getting requests via e-mail and on Post-it notes for holiday time. Several staff members have come to you stating that they “never” seem to get their requests for holidays. Discussion among the staff members is creating dissension and conflict.

1. Discuss the potential impact of this problem on you and the unit staff.

2. Describe a minimum of one positive consequence and one negative consequence of this conflict.

3. Select a model of conflict resolution and explain how you, as a nurse manager, might resolve this conflict.

Peer Discussion 1 (Kristen)

Recently, I experienced a communication error between a nurse and a patient. The type of communication was verbal communication. “Verbal communication is a simple form of communication but can become complex when trying to understand the cues conveyed by choice of words” (Pearson, n.d.). During the discharge instructions, the nurse was going over what to expect such as the symptoms the patient may experience. In a rush, the nurse told the patient that if she continued to experience COVID symptoms to come back to the ER. The nurse was supposed to inform the patients that If the symptoms get worse then to come back. The nurse did not clarify that and the patient came back 2 days later stating she was feeling the same and had no new symptoms but came back because the nurse informed her to come back if she was still experiencing symptoms. This communication could have been prevented if the nurse took the time to go over the discharge instructions with the patient and discussed how long the patients symptoms may last and if symptoms get worse to come back. When we complete discharges, we are supposed to go over the discharge and repeat the key points at the end. “It is helpful to repeat information so that the sender can verify that the correct message was received” (Pearson, n.d.). My workplace recently started a new discharge step that included calling the patient the day after discharge and going over the discharge manager. The discharge manger includes going over discharge instructions again and seeing if the patient was able to fill prescriptions and get a follow up appointment with their primary physician or a specialist if needed. This allows any clarifications or new questions the patients may have to be answered. NUR 435 DSN Wk 8 Communication Skills for The Healthcare Professional Discussion and Responses

Peer Discussion 2 (Tina)

Communication is necessary in many industries, but it is essential for the medical world. Poor communication, miscommunication, and deprived contact can lead to medical errors, such as treatment or diagnostic errors (Day & Beard, 2019). Quality communication is an essential part of the health care sector. Effective communication between nurses and carers is essential for safety and patient outcomes.

The impact of poor communication on medical errors may be more significant than indicated in the report, as it focuses only on cases of medical malpractice. It has been estimated that medical staff have poor communication during the transfer of patients, which causes 80% of serious medical errors. The following situation is an excellent example of how communication can give patients better results. NUR 435 DSN Wk 8 Communication Skills for The Healthcare Professional Discussion and Responses

The first parties included anesthesiologist and nurses. The hospital recently purchased a new blood handling device during surgical procedures. To date, the anesthesiologist has been trained and used in six surgical procedures. The device is used in this surgical condition. In this case, the device comes with two units of blood. When the anesthetist used the last unit of blood, the patient suddenly developed cardiac arrest. Anesthesiologist Dr T could not suspect air embolism and was unable to take appropriate resuscitation measures. The communication technique could not be practiced appropriately.

The lapse in communication could have been prevented, and the patient would have been successfully resurrected, but the oxygen supply to the brain was limited for a long time. It was realized that he was exposed to severe brain damage in the following days due to lack of oxygen. He was sent to a talented nursing home, hoping to return to work. There were many causes of air embolism, but communication between the anesthesiologist, the nurse and the surgeon was blocked, and new tools to understand the root causes of the side effects could not be understood.

Using The PICOT Model I will explore the following question: Can clients enrolled in a rehabilitation program and allowed to live in a sober living house, including free room and board, daily group therapy, weekly individual therapy, bi-weekly medical assistance programming and transportation for one year complete and addiction program without periods of relapse. Denver School of Nursing Smoking Cessation Intervention Discussion

P

What is the population of interest?

Opiate dependent clients

I

What intervention or issue do you want to consider?

Provide a sober living house, including free room and board, daily group therapy, weekly individual therapy, bi-weekly medical assistance programming, and transportation

C

What is the current nursing practice with this population, or what is the comparison practice? What is the context of the study?

Patients that are battling with opiate dependence but remain an active participant in the community can participate in rehabilitation but still live their day-to-day lives.

O

What is the outcome of those patients receiving the intervention?

Patient will successfully complete programming without relapse.

T

Over what period will the intervention be considered?

One year

I would go about finding the answers to this by having two groups.

Group A-provided with sober living and additions for one year while attending outpatient rehab.

Group B- Not provided with housing, including free room and board, daily group therapy, weekly individual therapy, bi-weekly medical assistance programming and transportation.

The research method would be both qualitative and quantitative. Denver School of Nursing Smoking Cessation Intervention Discussion

Not only will the patients, urine drug screen results, individual sessions with counselors, group therapy attendance and complain with medical appointments be tracked, but also meaningful participation.

A patient may not just attend programming and not participate.

According to The College of St. Scholastica, nurses can aide in successful completion from an outpatient program by monitoring patient compliancy with medical appointments, rendering urine drug screens to monitor sobriety and provide education regarding opiate cessation medications(The College of St. Scholastica, 2018) .

Outpatient treatment can be the best alternative for these with opiate addiction but are still living their everyday life. Lack of housing, food, transportation can be a risk factor foe relapse and removing those factors has been proven to aide in the relapse free completion of outpatient rehabilitation(n.d.)

Discussion 2

PICOT stands for Population, Intervention/Indicator, Comparison, Outcome, and Time (“Nursing: PICOT”, 2020, para.1). These are attributes of a good research question. For example, one might ask “Are biological children of obese parents at increased or decreased risk for obesity compared to adopted children with obese parents?” This addresses the population (children), indicator (adopted children of obese parents), comparison (biological children of obese parents), outcome (risk), and time (childhood).

To find evidence that would answer the question, the nurse could access already existing medical records or conduct their own survey, collecting samples from families with obese parents, and some with biological children and some with adopted children (or a mix). Being able to conduct their own survey is more likely to have more accurate results, and also allows for the possibility of doing further research into lifestyle. Using statistical analysis, the nurse could then observe the likelihood of a child to be obese if they are adopted or biological children of obese parents. This would involve a quantitative research method (using BMIs and weight as the numerical research values), and an ethnographic design (observing people in their normal states and recording relevant values such a diet and exercise within the family lifestyle). Ethnography is the “study of people in their own environment through the use of methods such a participant observation and face-to-face interviewing” (“NPS Ethnography”, 2020, para. 1).

The New England Journal of Medicine cited a study done on this very topic, and concluded that “Genetic influences have an important role in determining human fatness in adults, whereas the family environment alone has no apparent effect” (Stunkard & Hanis, 2012, para. 1).

Another study concluded that “Comparing fatness levels of genetically unrelated ‘family’ members and measuring synchronies in fatness change among family member who live together, it is clear that fatness and obesity are both acquired in the family context” (Garn, 2008, para. 2). They conclude that obesity is more tied to genetics than family lifestyle.

My own personal experience is further confirmation of this trend. I have two parents and 8 siblings, and not a single one is overweight. In fact, most of us tend to be a little underweight, especially as children and young adults. While none of us are adopted, our lifestyles are dramatically different from each other. For example, several of my siblings take health very seriously, and they run marathons together and focus on whole foods diets. Others of us have desk jobs, never exercise, eat fast food, soda, and processed foods regularly. But we all maintain a pretty average weight. Peer Discussion 1 (Paul Joe)

The roles of an interpreter and translator involve communication across cultures and they are both considered language services. The method of delivery is the main difference between an interpreter and a translator. While an interpreter is a person specially trained to convert oral messages from one language to another, a translator is a person specially trained to convert written message from one language to another (Interpreter vs translator, n.d para 2 & 3).

To provide cultural sensitive nursing care, an interpreter and a translator convey a patient’s message in the original context without any alteration, and taking into consideration cultural context. Also, interpreters and translators are usually not relatives of the patients. However, they are trained medical professionals guided by the code of ethics.

At my work place, interpreter services is readily available. Every unit at the hospital I work has an I-pad for interpreting services. So, after selecting a language and with a click of the call button, a live video conversation starts between the patient, nurse, and interpreter.

Since it involves oral conversion from one language to another, the service at my work place is interpreting. The interpreting services have helped overcome language barriers, and have met the need of patients and nurses 100 percent of the time. NUR 370: Week 8 Discussion: The Professional Interpreter

Peer Discussion 2 (Elizabeth)

Interpreters and translators enables individuals who would normally have difficulties communicating with each other the means to do so. The main difference between an interpreter and a translator is an interpreter converts verbal messages from one language to another while a translator converts written messages from one language to another. For an example, when someone is speaking to an individual who speaks a different language, the interpreter will “interpret” the message that is being conveyed while also expressing the information in the same way that it is being delivered. On the other hand, a translator will “translate” a text into the desired language so both parties are able to communicate.

“While the “delivery method” (e.g. oral or written) may differ between the two professions, both reflect the cultural terms, expressions, and idioms that provide meaning to the content. Both must capture any expression or nuance in the meaning of the original content. In certain instances, concepts will have no linguistic equivalent, but the interpreter and translator’s job is to find an equivalent way of conveying the message with accuracy and completeness” (Refugee Health TA).

I currently work as a Registered Nurse Care Manager at an insurance company and the majority of my time I am doing assessments with patients via telephone due to Covid-19. Many of my patients do not speak English therefore a 3rd party interpreter is involved in the calls. These interpreters are imperative to this job because without them many nurses would have no way to communicate with these patients. Also, when I am conducting my assessments, if my patient does not understand a word that was sad the interpreter will ask, “does interpreter have permission to explain the meaning in more detail”? These interpreters have a vast understanding of medical jargon and are able to elaborate more on a word in the case a patient does not fully understand.
