The analysis of position papers that your interprofessional team presented to the committee has convinced them that it would be worth the time and effort to develop a new policy to address your specific issue in the target population. To that end, your interprofessional team has been asked to submit a policy proposal that outlines a specific approach to improving the outcomes for your target population. This proposal should be supported by evidence and best practices that illustrate why the specific approaches are likely to be successful. Additionally, you have been asked to address the ways in which applying your policy to interprofessional teams could lead to efficiency or effectiveness gains.

INSTRUCTIONS
For this assessment you will develop a policy proposal that seeks to improve the outcomes for the health care issue and target population you addressed in Assessment 1. If for some reason you wish to change your specific issue and/or target population, contact your FlexPath faculty.

Propose a policy and guidelines that will lead to improved outcomes and quality of care for a specific issue in a target population.
Advocate the need for a proposed policy in the context of current outcomes and quality of care for a specific issue in a target population.
Analyze the potential for an interprofessional approach to implementing a proposed policy to increase the efficiency or effectiveness of the care setting to achieve high-quality outcomes.
Communicate proposal in a professional and persuasive manner, writing content clearly and logically with correct use of grammar, punctuation, and spelling.
Integrate relevant sources to support assertions, correctly formatting citations and references using APA style.
ADDITIONAL REQUIREMENTS
Length of proposal: 2–4 double-spaced, typed pages, not including title page or reference list. Your proposal should be succinct yet substantive.
Number of references: Cite a minimum of 3–5 sources of scholarly or professional evidence that supports the relevance of or need for your policy, as well as interprofessional considerations. Resources should be no more than five years old.
APA formatting: Use the APA Style Paper Template linked in the Resources. An APA Style Paper Tutorial is also provided to help you in writing and formatting your analysis.

Three health disparities that can be prevented or managed with regular physical exercise are: Hypertension (HTN), Coronary Artery Disease (CAD), and Obesity. According to statistics found in Medical Surgical Nursing, in the United States, one in three adults have HTN (p.738), about 610,000 people die of heart disease in the United States every year–that’s 1 in every 4 deaths (Heart Disease Facts, 2017) and over one third of Americans are obese; however, this statistic jumps to 66% when considering how many are overweight in addition to obese (p.946).

As a nurse many factors need to be considered when incorporating health promotion measures such as exercise and physical activity into our clients’ lives. The Health belief model (HBM) is a theory used to predict and explain a patient’s health behavior. It is based on three assumptions: the patient’s readiness to change, their perceived need to change, and the client’s perceived benefits of the health change (Edelman, Kudzma, & Mandle, 2014 p.219) A program can be designed to meet the patient’s motivation level or their ability to perform certain exercises or activities. One thing I would impress upon all the patients is try to walk, bike or take the stairs whenever possible; for some, this maybe a big change. Another strategy I would try, is find out what outdoor activities they like to do or have them try a new sport. By becoming interested in a new hobby or activity they are becoming more active and don’t really think of it as “work.”

The current guidelines recommended by the American College of Cardiology/AHA is at least 30 minutes of moderate activity on most days of the week. Examples provided of moderate activity include: brisk walking, hiking, biking, and swimming. Regular physical activity contributes to weight reduction (Lewis, Dirksen, Heitkemper, Bucher, & Camera, 2011 p.767, 768).

The importance of exercise in protecting human health cannot be underestimated, and its value to middle-aged adults is particularly evident. Regular exercise provides adults with a host of benefits, including fighting depression, heart disease, diabetes, and many different kinds of cancer: especially colon and breast cancer (Harvard Medical School, 2018).

According to Harvard Medical School (2018), regular exercise for only 30 minutes daily can prevent the onset of heart disease and reduce the chances of death for those who have already developed heart disease. In addition, regular exercise reduces the incidence of hypertension and diabetes (Harvard Medical School, 2018). For those with developed diabetes, regular exercise can manage the symptoms of the condition (Harvard Medical School, 2018). Regular exercise is an excellent remedy for depression and the protection against cognitive decline in later years (Harvard Medical School, 2018). It also strengthens the bones and prevents the loss of muscle density (Harvard Medical School, 2018).

When considering what kind of activities healthcare providers should recommend, the age, health status, and physical capability of each patient should be considered. Recommended exercises should include weight training; flexibility exercises; cardiovascular activities, such as running, brisk walking, and swimming; and biking. Patients should be encouraged to burn an additional 700 to 1000 calories during weekly exercise.

Education is the best approach to encourage patients to participate in exercise because there is a better chance of commitment when patients understand the value of exercise in protecting their health in mitigating the impact of existing health conditions. In addition to the well-documented beneficial health effects of exercise, I can personally attest to its value in fighting depression, managing stress, and creating a general sense of well-being.
