STUDY OF DIVERSITY AND THE CHILDBEARING FAMILY Purpose: Students will develop an appreciation for the uniqueness inherent in membership in a cultural or religious group and suggest strategies for considering that uniqueness in providing holistic nursing care to the childbearing client in the context of her family. Emphasis will be placed on those diverse groups with common representation in the metropolitan middle Georgia practice area. Objectives: As a result of this study of diversity and childbearing, the student will be able to: 1. Compare and contrast the dominant cultural values in the Middle Georgia area as they relate to the childbearing family. 2. Examine how a family’s cultural heritage with its inherent traditional health beliefs and practices affects contemporary culturally diverse families. 3. Given a real or simulated client representing a culturally diverse group, suggest how the nursing care plan should be altered to reflect her cultural uniqueness. 4. Examine beliefs within the major religious groups commonly represented in the metropolitan Middle Georgia area that can affect health care. Instructions: Each clinical group will examine a selected cultural or religious group of their choice or from the list below and will develop a power point presentation and post in the discussion section in D2L. Students may explore their assignment in the literature or through an interview with members of that particular group. The report should include information about the following: 1. Traditional health beliefs and health practices 2. Beliefs that affect family life & childbearing 3. View on contraception 4. Views on pregnancy 5. Beliefs about childbirth 6. Dietary practices • The power point should: o be at least six slides but not more than ten slides answering the above information o Include at least three (3) references in APA format published within the past 10 years for your culture or religion o Be posted in the discussion section in D2L o Include a paragraph about how the findings would affect the nursing care planning for that particular group in question. 1 • • • Each clinical group will present their culture report during class presentation day. You are encouraged to remain professional and adhere to University guidelines but you can be creative and unique during your group presentation. Jessica works 50 hours a week at a local restaurant and Marco has been contracting side jobs in construction. Six months before their move to an apartment, Jessica finds out she is pregnant. Four months later, Jessica and Marco arrive at the county hospital, a large, public, nonteaching hospital. A preliminary ultrasound indicates a possible abnormality with the fetus. Further scans are conducted and it is determined that the fetus has a rare condition in which it has not developed any arms, and will not likely develop them. There is also a 25% chance that the fetus may have Down syndrome. Dr. Wilson, the primary attending physician, is seeing Jessica for the first time, since she and Marco did not receive earlier prenatal care over concerns about finances. Marco insists that Dr. Wilson refrain from telling Jessica the scan results, assuring him that he will tell his wife himself when she is emotionally ready for the news. While Marco and Dr. Wilson are talking in another room, Aunt Maria walks into the room with a distressed look on her face. She can tell that something is wrong and inquires of Dr. Wilson. After hearing of the diagnosis, she walks out of the room wailing loudly and praying aloud. Marco and Dr. Wilson continue their discussion, and Dr. Wilson insists that he has an obligation to Jessica as his patient and that she has a right to know the diagnosis of the fetus. He furthermore is intent on discussing all relevant factors and options regarding the next step, including abortion. Marco insists on taking some time to think of how to break the news to Jessica, but Dr. Wilson, frustrated with the direction of the conversation, informs the husband that such a choice is not his to make. Dr. Wilson proceeds back across the hall, where he walks in on Aunt Maria awkwardly praying with Jessica and phoning the priest. At that point, Dr. Wilson gently but briefly informs Jessica of the diagnosis, and lays out the option for abortion as a responsible medical alternative, given the quality of life such a child would have. Jessica looks at him and struggles to hold back her tears. Jessica is torn between her hopes of a better socioeconomic position and increased independence, along with her conviction that all life is sacred. Marco will support Jessica in whatever decision she makes, but is finding it difficult not to view the pregnancy and the prospects of a disabled child as a burden and a barrier to their economic security and plans. Dr. Wilson lays out all of the options but clearly makes his view known that abortion is “scientifically” and medically a wise choice in this situation. Aunt Maria pleads with Jessica to follow through with the pregnancy and allow what “God intends” to take place, and urges Jessica to think of her responsibility as a mother. © 2015. The health promotion models are used to explain and to promote health behavioral changes. Bandura’s self-efficacy theory of behavior change is one the theories that promotes behavioral changes. This theory was introduced by Albert Bandura in 1977 (GCU, 2018, para. 30). This theory is based on the ability that each person has to achieve goals by believing that they have the ability to accomplishing their goal. This theory encourages to learn by repetition, reinforcement and symbolic modeling. For example people learn by watching something over and over and practicing a task multiple times. This is when repetition and reinforcement come into place. Another great example is when people start in a new job and the first couple of days they feel very lost but then it becomes a habit. Some barriers that can affect the ability to learn can be environment, cognitive status, age, desire to learn and culture. A patient’s willingness to learn can affect tremendously the learning outcomes. A patient that is willing to learn will listen, ask question and will value all the information that is given to then. Therefore, information will be retained. On the other hand, a patient that is not willing or ready to learn will not pay attention to the information that is given to him, therefore, he will not learn anything.
