A family is made up of persons linked by consanguinity, affinity, common residence or adoption and who depend upon one another for the fulfilment of certain obligations required for the benefit of one or all members. Family assessment therefore is not only relevant in the prevention of diseases but also very important in the promotion of health. (Edelman, Kudzma &Mandle, 2014). The importance of learning family assessment theories cannot be overemphasized. For instance, it has been suggested that nursing students who receive proper training on use of different theoretical frameworks during assessment of families tend to do much better much later in practice. (Lee, Leung, & Mak, 2012)

There are many theories which provide a framework for the assessment of the family. However, this student prefers the Calgary Family Assessment and Intervention Model, (CFAIM). This student believes this theory is the most effective and useful of all theories of family assessment because the CFAIM assesses family from different directions and focuses on communication and change. CFAIM has three branches-structural, developmental and functional. The structural aspect of the theory deals with family composition, characteristics and organizational structure of family. The developmental aspect is concerned with family genealogical mutation. The functional aspect assesses how family members interact with one another. (da Mata Ribeiro Gomes, 2014)

This student prefers the CFAIM because it assesses family from multiple dimensions in order to present a clearer picture. It stretches the role of the nurse from simply studying the family to assisting in the stabilization of family as a veritable way of strengthening the bond between members of a family and their community. (Χριστιάνα, Μαρία & Παναγιώτα,2013). Again, by studying family structure, the CFAIM assists in linking certain characteristics of a family under study to risk factors for certain conditions. For instance, it has been suggested that poor income, vast number of children and absence of a father figure places a child who is a member of such family at high risk for falls. (Aline de Souza, et al 2012)

Another important reason why this student believes this theory is appealing and effective is that it has effective applicability in family assessment in a variety of settings and situations. For example, it has been suggested that using the CFAIM in hospitals and other healthcare settings allows for a more in-depth understanding of the family structure of the patients being assessed. (da Silva Monteiro et al, 2016). This model has also been used to prove that the entire family dynamics, structure and organization in every family play crucial roles in its health dynamics and support system for members of such family. ( Sabino Cavalcante, Moreira Rodrigues, Mendes de Paiva, Mourão Netto, & Frota Goyanna, 2017).

Family is the nucleus of society It is the unit from where every other societal unit derives. This student hopes that the current efforts to more clearly understand the concept of family within different theoretical frameworks will continue.

Families are very different and constantly going through changes. Each family has their own unique structure and own ways of dealing with these changes. In order to understand a family and the risks associated to them, a nurse must use a framework assessment as a tool to collect data to better understand family structures and functions. For this reason, this student feels that Gordon’s Functional Health Patterns is a more useful guide to the nursing process in regard to assessment of the family. This particular theoretical perspective encompasses eleven functional health patterns enabling the nurse to comprehensively organize family information. These patterns include health perception-health management, nutritional-metabolic, elimination, activity-exercise, sleep-rest, cognitive-perceptual, self-perception/self-concept, roles-relationships, sexuality-reproductive, coping-stress, values beliefs. Patterns form the standardized format for family assessment using a systems approach with emphasis on developmental stages and risk factors (Edelman, Kudzma & Mandle, 2014). Assessment includes evaluation of dysfunctional patterns within families with corresponding details in one or more of the other interdependent patterns (Edelman et al, 2014). Gordon’s Functional Health patterns approach is the theory most appealing to this student because it serves as a more organized way to obtain relevant data. An organized approach is always best practice when performing assessments on families and an individual.

Some specific health problems that

can be positively affected by regular exercise include: hypertension, diabetes, obesity, mental health, and arthritis. Hypertension can be significantly improved by incorporating thirty to forty-five minutes of aerobic exercise 5-7 times per week. Aerobic activities that are recommended for the middle-aged adult include: bicycling, walking, and swimming. Engaging in regular aerobic activity has been shown to decrease both the systolic and diastolic blood pressure (Dimeo, 2012). According to the CDC, 1 in 3 adults suffer from hypertension; with the incorporation of regular exercise, the prevalence of hypertension can be reduced (2016). Arthritis is another disease that middle-aged individuals deal with; patients who suffer from arthritis have shown improvement with the incorporation of regular exercise and activity. Previously, physicians would encourage patients with arthritis to rest their joints, but physical activity has been proven to reduce pain, increase function, and improve mood, mobility, and quality of life (CDC, 2016). Activities that should be encouraged in the middle-aged adult suffering from arthritis can include: yoga, tennis, and golf. Mental health has shown improvement with the incorporation of a regular exercise, due to the release of endorphins. Mental health disorders such as anxiety and depression have shown improvement through adding regular exercise into everyday life (Sharma, 2006). Patients who use physical activity in their daily lives have reported increased self-esteem, and is often used in the treatment plan of patients with depression (Edelman, 2014).

Nurses must promote physical activity in all patients, not just middle-aged adults, or patients who already are showing signs or symptoms of a disease process. Using the patient’s interests as a motivating factor is essential in achieving positive outcomes. Utilizing the patient’s interests will increase the likelihood that the patient will be motivated to engage in regular exercise and other activities and continue the routine into the future. Assessment of barriers and hesitation is also important when developing a plan of care for each individual patient.

Regular physical exercise and activity performed by the middle-aged adult can reduce the risk of health problems including heart disease, type 2 diabetes, and arthritis. The prevalence of heart disease in the U.S equals 610,000 deaths per year (Centers for Disease Control and Prevention, 2017). Type 2 diabetes contributes to 90-95% of all cases and in 2015 1.5million new cases where diagnosed in the U.S in adults 18 years or older (Diabetes.org, 2017). The CDC estimates by 2040 there will be an average of 26% of adults over the age of 18 that will have been diagnosed with some form of arthritis including rheumatoid arthritis, fibromyalgia, gout, and lupus (Centers for Disease Control and Prevention, 2018). The nurse can perform a physical activity screening to determine what activities the patient enjoys participating in and how much exercise a person is incorporating into their daily lives. The CDC recommends participating in low intensity, moderate aerobic exercise for 150 minutes per week to reduce the risk of heart disease, type 2 diabetes, and arthritis. An approach the nurse could use to gain cooperation from the patient would be helping them to incorporate the desired amount of exercise into their daily living through activities and exercise they enjoy participating in. Nurses can also utilize the National Physical Activity Plan located at www.physicalactivityplan.org, which encourages health care workers to promote physical activity guided by a comprehensive, evidence-based strategic plan (American Journal of Nursing, 2015, para. 10). This plan is designed to help health care workers promote physical activity by making this topic a “vital sign” that health care workers assess and discuss with their patients, establish that inactivity is a treatable and preventable health conditions, provide health care workers with physical activity education, and use the health care system to promote physical activity in hopes that all American’s will become physically active and work, live, and play in environments that support an active lifestyle (American Journal of Nursing, 2015).

Statistical Interaction is when two or more variables are placed in a situation to see if there is correlation between them. One variable needs to be able to react with another variable to determine if there is any statistical interaction (Statistics How To website, 2016). A statistical interaction is like having one issue and seeing if it can be treated by two different types of treatments. For example, in my professional field would be in between two factors such as variable “one” patient being treated for Chorioamnionitis with Ampicillin and Gentamycin and variable “two” patient being treated with Zosyn which patient response to the different treatment is progressively better than the other within 48 hours. You would commonly run into situations of interactions in the medical field but also in everyday life. For instance, weather is one of them. The dependent variable would be a weather front, the independent variables would be location of the weather front and whether it is a warm or cold front. That would determine the activity of the front systems and predict its path.

The levels of research evidence identified will help you determine the quality of the evidence that is available for practice. The best research evidence generated from systematic reviews, meta-analyses, meta-syntheses, and mixed-methods systematic reviews is used to develop standardized, evidence-based guidelines for use in practice. (Grove 2015) The levels of the research evidence are a continuum, with the highest quality of research evidence at one end and weakest research evidence at the other.

Strongest research evidence –

1.Systematic review of experimental studies (Use of RCTs)

2.Meta analysis of experimental RCTs and quasi experimental studies

3.Integrative reviews of RCTs and quasi experimental studies

4.Single experiential RCT.

5.Single quasi experimental study.

6.Meta analysis of correlational study

7.Integrative reviews of correlational and descriptive studies

8.Qualitative research metasynhesis and meta-summaries

9.Single correlational study

10.Single qualitative or descriptive study

11.Opinions by respected authorities based on clinical evidence, reports of expert committees
