Re:Topic 2 DQ 2

The general public is always very diverse in their thoughts and views as evidenced every day when people voice their political views or discuss food choices or banter back and forth about what cars they choose to buy. Nurses unfortunately, are not on any different spectrum when it comes to how they are perceived. Many people think that nurses are just transporters of patients or gatherers of supplies. They have learned all they know about nurses from watching hundreds of hours of television shows over the years where they show the doctors doing the work that the nurses usually do. Some people think that nurses are only there to take you to the bathroom or help you with a shower or bath. They have no idea what a Certified Nurse’s Assistant is or what duties they perform. Many times, throughout the work week in the Intensive Care Unit phone calls are received or friends or family of patients visit, and they want to talk to the doctor. Those requests are triaged and sometimes they can be valid because of a very complex situation but usually more often than not they are simple questions that the nurse or charge nurse is able to answer. That is something that is always mind blowing and makes one realize what little information that a lot of people think nurses know.

Television plays the biggest role in people’s thoughts and understanding about what nurses do. Television has very unrealistic situations and very rarely shows nurses doing the actual work that they do on a daily basis. On the daytime show, “The View” one of the hosts mocked a nurse during a pageant asking why she was wearing a “doctor’s stethoscope.” This sparked national attention and debate. Many nurses took a stand and the program ended up losing some of their sponsors and had to issue an apology.

Clothing also creates a huge amount of confusion over what a nurse’s role is. Scrubs are worn by anyone and everyone that chooses to wear them whether they be a food services worker, a housekeeper, or a biomedical engineer. Anyone wearing a white coat that is male is usually perceived as a doctor, even when they have already verbally identified themselves to the patient and family as a nuclear medicine technician or echocardiogram technician. Medical assistants in physician’s offices are often called “nurses”. Many of them have a certificate in medical assisting from a college but this even is not always required for them to work in a physician’s office as it is up to the physician and office manager’s discretion.

In order to educate the general public about the professional nurse’s role it would take a lot of work and a large number of nurses coming together to create change along with consistency of follow through year after year. Some suggestions (Summers & Summers, 2016) to start the education process would be for the nurse to project a professional image in all interactions. This would entail that when you meet someone you would introduce yourself as a nurse and include your last name as physicians do. Also, when performing your nursing assessment, you could describe your assessment and interventions as you perform them. This could help patients and family members to understand your professional knowledge and ask questions. As referenced above, clothing can play a big part in what the public may think a nurse’s role is. Cartoon characters on scrubs can seem childish and unprofessional unless worn in the correct limited setting. Nurses should work to maintain professionalism in their own attire, appearance, grooming and hygiene.

Media portrayal of nurses has a huge effect on the public’s thoughts and knowledge of the nursing profession. Hospitals often have communication departments that highlight physicians on their bulletins or webpages. One way to promote the nurses in a facility (Summers & Summers, 2016) would be to form a work group to increase awareness of nursing achievements and innovations and publish those on a bulletin or webpage that is seen by the public and/or other facility employees. Many times, the media can get things wrong just as they can also get things right. Nurses can submit feedback to media outlets by giving positive reinforcement. This can be done by phone calls, hard-copy letters, email, twitter hashtags or other social media commenting. The same strategy can also be used when negative nursing portrayal is observed.

Sample theory was developed to find the most effective way to get data from a selected process, group of people, events, objects, or other elements. The sample selected is a study that should identify a selected population like, insulin dependent diabetics, gastric bypass patients, All total knee orthopedic surgery patients. In most cases it is impossible to get data from every person that has ever received this treatment, so sampling was designed to get the most accurate information to reflect the population as a whole. Some of the key concepts include eligibility criteria, the population, target population, accessible population, and sampling method or plan. One that comes to mind is that we have to summit trauma data to the national trauma data bank, this is where they have criteria points that the chart must meet and from there specific data points for the different types are asked. This is like the time to pain medication for all long bone fractures.

We do bedside report at our hospital because evidence-based practice shows a decrease in errors during shift report. Involving patients and families in bedside shift report helps reassure that nothing is being left out or unaddressed. Being able to assess the environment and patient better. Bedside report enables accurate and timely communication between nurses includes. The patient care is paramount to the delivery of safe high-quality car (OJJIN, 2018). In the literature review stated although beside report is not a new concept many studies support the positive impact of bedside report. The results of doing bedside report at my workplace have improved to decrease falls, better stratification of the family, patient confidentiality, by giving report in the room instead of the nurse’s station, able to address the neuro states, see if anything else needs to be addressed, check orders and make sure the patient request are fulfilled. Bedside report has improved communication and team work in our facility.

Euthanasia is the act of deliberate ending of a patient’s life at his or her request or following the request of their families. In healthcare services the term is often referred as mercy killing as it purposes to hastening death of a patient who is suffering with chronic diseases and requesting relief of that pain (McLeod-Sordjan, R., 2014). In preaching of Roman Catholics, the sin of suicide or euthanasia is condemned as a murder and therefore it is described as mortal sin. It is based on the belief that God has supreme power over his creation and human put into suffering because of some purposes. However, Protestant gives different opinion. For instance, it is not the duty of a common man to limit life of an individual and the man should stop playing with God. On the other hand, others consider euthanasia as an acceptable thing as they consider pain and sufferings as purposeless, demoralizing and degradation of individual (Nair, U., 2012). In one of the survey conducted in Australia, it has been observed that 70% of the people support euthanasia. They believe that euthanasia is a compassionate and sensible act allowing doctors to deliberately terminate the life of patient at their request. Being a nurse, I am agreeing with this thought of deliberate termination of death of chronic patients. It is because of the fact I have seen many individuals’ sufferings with chronic diseases and do not have facilities of quality healthcare services. At that stage they seem helpless and gone through tremendous pain and sufferings. For these patients I can suppose this way of ending life.

1. Mucor is a mold found in soil, plants, manure, decaying fruits, vegetables and as a common contaminant of stored and processed food in the kitchen.( Mold and bacteria consulting laboratories). Mucormycosis is a serous but rear fungal infection caused by a group of mold called mucormycetes.( CDC). According to CDC, there are five clinical forms of mucormycosis but the most common one is Rhinocerebra mycormycosis which occur mostly in diabetic patient, nutropenic cancer patients and hematopoietic stem cell transplant or solid organ transplant recipients and pulmonary mycormycosis.

Similarly, other things that can put people at risk of developing mycormycosis according to CDC are prolonged corticosteroid therapy; skin trauma, burns, or surgical wounds; iron overload; intravenous drug use; malnourishment and premature infants.

Patients is likely to get infected with mucor “through inhalation, inoculation, or ingestion of spores from the environment.”(CDC).

When the spore is inhaled, it travel tthrough the alveoli and bronchioles of the lungs causing inflammation and pneumonia with clinical manifestation of fever, cough, chest pain and dyspnea.

Furthermore, medical treatment is with antifungal medication usually, Amphotericin B, Posaconazole or Isavuconazole. Surgery may also be required to cut affected tissue (CDC). Nursing intervention that will be helpful to patient include monitoring of vital signs and laboratory result adminitration of oxygen when needed, and keeping accurate record of intake and output.

2. llaboratory result The white blood count (WBC) is high 15,200 this shows that patient have infection secondary to inhalation of mucor which may have spread inside the body causing infection. Arterial blood gas is abnormal with high PH 7.50 , PaC02 is 25 and PaCO3 25. The three ABG shows alkalosis. The patient has respiratory alkalosis secondary to hyperventilation. Patient SPO 59 room air is low , oxygen should be given to increase the level and enhance respiration.

3. Medication and medical treatment.

Three mdications for the treatment of mycormycosis are;

Amphotericin B, Posaconazole or Isavuconazole (CDC). These medication attack the structure of and function of fungal cells. They inhibit the DNA synthesis of fungus there by preventing from spreading. Amphotericin binds fungal cell membrane and leaking cellular content. Surgery may also be required to cut away the affected tissue (CDC).

Finally, controlling immunocompromising condition should be attempted (CDC).

There are some concerns that PPIs may be overprescribed and may predispose patients to GI infections because of the reduction of the normal acid-mediated antimicrobial protection (Lilley, Collins, Harrington, & Snyder, 2011). New concerns have arisen over the potential for long-term users of PPIs to develop osteoporosis, due to the inhibition of stomach acid and the speculation that PPIs speed up bone mineral loss (Lilley et al., 2011).
